Health Policy

Parents must fill out the Annual Student Health Assessment and turn it in to the office on, or before, the child’s first day of school. The child's immuni law.  All immunizations must be up to date at the time school begins for the child.  The school will accord the parents a one-week grace period in which to provide current immunization records, after which the child will be unable to attend Ogden Valley Montessori School until his or her records are on file. 

For returning students, all records must be updated annually and parents must notify the administration when a child has received a new immunization. 

If parents choose not to have their child vaccinated, an Immunization Waiver must be provided from the local Health Department.

If your child requires medication, please have your doctor sign a clearly written order for such and give it to the child’s teacher, along with a copy of the Medication Release Form that is available from the office. Medications must be clearly labeled and in their original container.

The following is a list of symptoms to guide you in deciding whether or not your child should attend school. Please do not send him or her on days when any of the following symptoms are present:

Fever
Rash
Cough
Nasal discharge
Diarrhea
Vomiting
Sore throat

Children with any of these symptoms should remain at home until they are symptom-free for a period of twenty-four hours to decrease the likelihood of spreading an infection. A child who exhibits signs of illness while at school will be isolated in the office. Staff will call the parents, or other designated individuals, and request them to come and pick up their sick child immediately.

The school office should be notified as soon as possible if the child  contracts a communicable disease while at home so that notices of possible exposure may be posted for other parents.

Emergency Procedures

In the case of an emergency occuring during school hours, parents or other authorized persons will be contacted for instructions. If no emergency instructions can be obtained, the child will be transported by ambulance or authorized private vehicle to the closest hospital, and the instructions and recommendations of the attending physician will be followed (unless parents or guardians signed a refusal to consent to emergency medical authorization).
